I have a bunch of pillows rolled up in a blanket under my torso so my weight is not on my neck. Its mainly there to hold my head at just the right height for some deep throat training. I suppose a nice snug chest harness could be just as effective. My head could be held back with a rope from the harness gag to increase the vulnerability of the position.
