Here are some additions to the DIY Humbler from the DIY section, that add wrist, ankle, and collar attachments to the humbler.

The ankle loops are self explanatory, and cuffs, shackles, etc could be used.
The rope and cloth loop pull the head forward (safety note: fixed length loop and forward pull).
The wrist attachments are a little more complex. They include a wrist loop, 1/8 inch Rope Ratchet tightener, small NiteIze S-biner, and another wrist loop with built-in ice bag release. This loop is a braided shoe string with a brass ring and small nylon bag that goes through the ring then has ice cubes added. The Rope Ratchet usually comes with a small hook at the top, which can be loosened by forcing a screwdriver or small chisel through the loop, and removed.

The cords from the Rope Ratchet go through the humbler tube (preferably before the U-bolt is attached around the genitals) and the cord that tightens is attached to the first wrist loop. Both hands can be used to wrap the other loop and insert ice cubes in the bag. There should be enough slack to clip the S-biner onto the wrist loop, then the first hand pulls the other end of the ratchet cord to pull the ratchet and S-biner into the humbler tube so the wrists are fastened to the ends of the tube.
There are many possible variations: collar, chain through tube with cuffs at ends, longer ankle connections to allow some walking, etc.
