That's just a bit too many "just rights" to reassure me about sticking that inside someone's butt. Whenever one is relying on something that is assumed to be "just right", there's always something that is bound to go wrong.

Unless you try to extract it while it's extended.

I think the recommended extraction on sites selling Pear devices is to retract it to a slightly open position. Wide enough gaps to not pinch but narrow enough to reasonably expect a person who can handle it in the first place to be able to get it out. (I think both variants I came across were screw adjusted - put it in, turn the handle and it opens - though that might have just been the hand crafted replica of a medieval version)

Regardless, it might be a partners toy only - even with the added precision of a screw adjustment, it might not be worth the risk, given most people have trouble looking there without an assortment of mirrors screwing up the size interpretation.
