Not during bondage...but while on a climbing instructors course we were being taught how to teach people to use a belay device called a grigri. the session started with us jumping off the wall from about 20ft up with a few feet of slack on the rope so we knew exactly what the device's limits were, after one drop I somehow managed to fall so that I was only eight feet off the floor with my partner two feet up in the air attached to a sand bag. He tried opening up the belay device to lower me down but it wouldn't open so he let go off the rope and tried with two hands - whack. I droped instantly onto my back and it hurt like hell. Fortunatly I had a helemt on so didn't hurt my head much but the bruising on my back the next day was terrible.
Lesson: do not let people suspend you in the air unless they know exactly how all of the gear works and how to get you down safely if anything breaks.
