I just remembered, I did used to hang the ring from my old collar to position it at the correct height behind my back. This was more useful when I just did a strap around the arms. since I have been using elbow cuffs the strap is self locating to the rings on the cuffs.

If you want to use rope, what about wrapping several coils around each bicep and tying them off with a loop or a ring and then run the elbow loop through those.
